Successful Community Appointment Day: A New Era of Patient-Centered Care

At College Green Medical Practice, we’re always seeking innovative ways to ensure our patients receive the best possible care. On 9th October, we were thrilled to partner with the Royal Orthopaedic Hospital and Queen Elizabeth Hospital to deliver a hugely successful Community Appointment Day at Cocksmoor Woods Leisure Centre. This initiative brought a fresh, personalized approach to healthcare, demonstrating the power of collaboration between providers of nhs healthcare.

A Personalised Approach to Care

The concept behind Community Appointments is simple but powerful: patients are at the centre of their own healthcare journey. This event allowed attendees to express their concerns and share what matters most to them, enabling healthcare providers to offer tailored treatments that meet their individual needs.

The day was designed for patients on waiting lists for physiotherapy, as well as those from College Green Medical Practice who required further support. Attendees were given the opportunity to meet face-to-face with physiotherapists and connect with a variety of community and voluntary organizations that provide essential support services.

High Turnout

Over 250 patients attended, highlighting the eagerness of the community to take charge of their health. The dedicated team from College Green Medical Practice, included Dr. Chris Yan and FCP Enxhi Kumati, and with non-clinical support from Clair King and Karen Antcliffe. Building a Stronger Support Network

The success of the event was bolstered by the participation of key partners and organizations. Attendees were able to access guidance from groups such as Age UK, Versus Arthritis, Nuffield Health, Citizens Advice Bureau, and The Active Wellbeing Society, to name just a few. There were also talks on arthritis and pain management, providing invaluable insights for those managing chronic conditions.

This holistic approach to care shows that healthcare doesn’t stop at medical treatment—building a network of support through community and voluntary services is key to patient wellbeing. From pain management to weight management, mental health support to lifestyle advice, the event brought together all the resources patients need to thrive.

Looking Ahead

Community Appointment Day marked a significant step forward in how we deliver healthcare. At College Green Medical Practice, we are proud to have been part of an event that highlights the importance of personalized care, community collaboration, and patient empowerment.

The feedback from our patients has been overwhelmingly positive, with patients reporting that they found the day highly informative and beneficial. We’re already looking forward to future collaborations and initiatives that continue to put our patients first. We extend our thanks to everyone who made the day a success, including the dedicated teams from the Royal Orthopaedic Hospital, Queen Elizabeth Hospital, and all the supporting organizations.
